Any position within a Michigan-based long-term care setting is eligible, including clinical, administrative, support, and ancillary roles.
Each recipient receives a commemorative trophy, a $1,000 monetary award, and the Health Care Hero Purple Cape.
A completed nomination application must be submitted electronically and include: nominee details (name, title, facility, and contact info), nominator contact information, and a 300–600 word essay.
Provide specific examples of outstanding performance, a positive and uplifting attitude, respectful interactions, what differentiates the nominee from peers, and actions that go above and beyond normal responsibilities.
All application materials must be typed and submitted electronically through the website.
Recipients are selected by a committee appointed by the Healthcare Excellence Council, with a process designed for fairness, consistency, alignment with program values, and representation across long-term care communities.
